Eh, if you want to actually get strict, you can IP ban + ban the IPs of known proxies + use anomaly detection to catch players like Karl that bypass IPs. Combine that with a min level of 20 to post on non-Help forums, and you've got a system that works pretty damn well. It's a solved problem, just that good solutions are require some time and planning to replicate.
Even when bans are easy to bypass, they create inconveniences and discomfort so while they're not perfect, they still act as deterrents. A system doesn't need to be 100% effective to be worthwhile; often you can just prop up minor barriers and stop 95% of cases you care about.

I'd be willing to bet a lot that people on that list are a couple orders of magnitude more likely to be banned than those that aren't. Even with the presence of a few people that will probably never get banned.
