I think you raised an important issue, in that the term "fake news" is used so much that there is little specificity on what it actually means.
I think there is a large spectrum, from (i) knowingly printing false facts and purporting as news, to (ii) publishing stories that are unverifiable, anonymous sources, etc, and purporting as if they are true, to (iii) allowing your internal bias affect you from reporting fairly, while pretending to be unbiased.
Here are a few examples: (i) -NYT Editorial clearly linking Sarah Palin to assassination attempt, and basing the entire opinion piece on this false fact. -A site imitating an actual news sites that publishes fake news articles (less than tabloid level) and utilizing social media algorithms to promote those stories. ---Yes, its sad that I had to include the NYT in the same category as a tabloid article.
(ii) -In advance of meeting, unnamed source states that Trump will not discuss election hacking with Putin... proven false hours later. -In advance of testimony, unnamed source stating FBI director will state one thing, proven false hours later when FBI director testifies the opposite of that one thing. ---Both of these are CNN.
(iii) -When 52% of population states they are in favor of A, and 42% of the population states they are not in favor of A, picking a headline that states: "More than 4 in 10 Americans say that are not in favor of A" -The Washington Post did this.
So I won't go as far to say that, "News Source A is fake news," but there is a repeated pattern of promoting "fake news" from these agencies which undermines their credibility and trust.
"Drip, Drip, Drip, Drip, Drip" says Podesta.
So far, the most recent drip about DJT's emails actually could be something. Yet I've lost much trust that the news agencies cannot report verifiable facts fairly and without bias.
Often times, Anti-Trump stories get debunked well after the shock of the headline wears off that caused the initial drips. While the story is debunked, all most people can remember was that there was a drip, not what caused it or if it was real.
Now, we have an actual drip which may be caused by a real problem. Its much too early to determine if this drip will be cast away like the previous drips, but the credibility of the media is already shot in the eyes of many. So good luck trying to convince a majority of the people that this is a real drip, and not one falsely put out by fake news.

@taylor, I've already explained that in my post, which you seemed to have ignored.
And to expound on the prior explanation, there are too many people that are just not smart enough to distinguish what is improper reporting of news and the truth. They already bought into the narrative and bias perpetuated by Washington Times, CNN, NYT, and the like.
When presented with sound facts and opinions based on those facts, the stupid people just ignore it. I think because they've already bought into the narrative and bias and lack the cognitive ability to understand something that challenges their preestablished opion.

I think you're missing the point, Ox. The idea is that all the news industries right now deal with pretty heavy bias. Of course, you have to get informed somehow. The best thing to do is watch multiple sources from different political angles and put together the evidence for yourself.
People who rely on only one or two news sources for all their info are usually the ones who end up the most brainwashed.

Fake News should mean simply that- a story that is intentionally falsified or exaggerated, or based on unreliable sourcing.
Unfortunately, Trump people use it to mean "news that I want to deride because it paints the president in a negative light." In other words, Trump is the Great Good and the Media are the Ultimate Evil. Unfortunately instead of trying to calm it down, Trump plays this to his advantage and calls the media the enemy of the people. He does this so when a legitimate story comes out (ironically, an email scandal), his supporters will only think that the media is spewing bullshit and trust only Trump. They will believe whatever lies he says simply because he is saying them and the "enemy of the people" is saying the opposite.
