can one conjoined twin drink and the other drive

I'd imagine the law would decide based on whether they share the same stomach or both get drunk. Either way, you can't have open alcoholic beverages in a vehicle.

I would think not @ Orannis, because the alcohol would still flow through the blood.

But would it be fair to punish them both if only one was guilty?

Yes. You drank while driving. They would be treated as one entity. I am not a lawyer, do not take this as legal advice, this is just my estimation.
