No, idle is becoming more and more popular and there are definitely some good players who would care, maybe not the top 3, but definitely some in the top 10.

I will see your "Isn't it unrealistic to think that people who score high on the ladders would care about Idle rewards?" and counter with: yes it is unrealistic, but isn't it even more unrealistic to think that Fizz would suddenly take a different approach to rewards?
He's made CW rewards like this (coins + various Idle rewards [powers & buffs]), then revamped the coin leaderboard rewards in a similar manner (coins + powers, no buffs), so I see no reason why ladders would be different. The intent is to draw interest of WZC players to Idle, and interest of Idle players to WZC
I also offer you an additional counter question - what other rewards can you think of that would be reasonable? Any beyond coins that makes sense? Premium colours doesn't make sense given they cost $14 for the simple ones and up to $100 for rare ones; rewards that highly valued don't make sense
And before you say "ah but the total value of the rewards for CW 1st place is quite high" -- ignore the cost of powers; powers are an artificially over valued way of seemingly providing high value rewards (1 ML alone is $7), but we all know MLs aren't actually worth $7 and no one in their right mind would ever pay $7 for a single ML power, but it feels impressive to hand them out and claim a reward of the list price value
But whether top players like or play Idle or not is irrelevant. They're playing what they like (WZC) and getting rewards they also like (coins); getting rewards they don't care about (Idle) is not a huge detractor
I wouldn't slag a fellow who gave me $10 and a digital coupon for a free bottle of hair spray; I'd never redeem the hair spray but the $10 is no less valuable to me

Why would there be new rewards beside a trophy suddenly? The only change i hope for and expect is an improvement of the rating system.
Oh i saw it in fizzers message now. So yeah, even if it's idle (most likely is), it won't matter at all, cause it's a bonus, which you don't have to use if you don't want to.

If there's one thing we know about Fizzer it's this - if he does something, he does it right. So I do expect some major changes, for example:
- new rating system (obviously), hopefully Elo or Glicko2 but anything is an improvement really - improved statistics (for individuals and potentially even for clans) - take MTL as inspiration, but I have ideas for so much more - potentially the possibility to play a real-time ladder game (similarly to how you can play QM RT and MD) - rewards (well, he said that), what they're gonna look like idk, there's nothing I could think of besides idle - improved mechanics to earn the trophy - better UX funnel to guide (new) players to the ladders - improved history of the ladders (let's say I want to know the standings from March 21, who was first there) - auto-detection for stalling (will push a notification to the player reminding them to surrender if the game recognizes the player is down 5 vs 20 income or something like that) - more achievements (there's so much potential here)
I don't expect all of these to happen, but I hope for most of them.
